DOCUMENTS REQUIRED for ACCIDENT REPORTING

1. Driver’s NRIC & Driving License

– Mandatory for the person driving at the time of the accident.

2. Vehicle Owner’s NRIC

– If the driver and owner are different individuals, the owner's NRIC must be provided.

– For company-owned vehicles, a letter on the company’s letterhead is required, stating that the driver is authorized to operate the vehicle.

3. Certificate of Insurance

– Must be presented during reporting.

• The vehicle owner must be present during the accident report.

• The vehicle involved must be present during the accident report.

• For company vehicles, the company stamp (chop) must be presented.
